For the first time in almost three decades, office and outpatient E/M codes will undergo extensive revision. New rules will eliminate history and physical exam as elements for code selection, allow physicians to choose whether their documentation is based on medical decision making or total time spent on the patient encounter, and recognize work involved in non-face-to-face services. Yet, according to a recent survey conducted by Medical Group Management Association, less than half of all medical practices are prepared for these changes which are scheduled to take effect on January 1.
